Tamoxifen, a cornerstone endocrine therapy for estrogen-receptor-positive breast cancer, is bioactivated by CYP2D6 to (Z)-endoxifen, its most active metabolite. A pooled population-pharmacokinetic analysis of six clinical studies (468 patients) quantified the variability of endoxifen exposure, simulated a model-informed precision dosing (MIPD) workflow that would cut the proportion of patients below the proposed 5.97 ng/mL (~16 nM) endoxifen target from ~22% to ~7%, and identified as unresolved future work the validation of the endoxifen exposure–response relationship for breast cancer recurrence and the demonstration in prospective trials that individualized dosing improves clinical outcomes; the same analysis reported that a large interindividual variability in endoxifen formation (~47% CV) remained unexplained by the covariates it identified. These questions remain open: retrospective threshold evidence has been contested by null reanalyses, no determinant set explains the residual variability, and although TDM- and MIPD-based individualization is feasible and has been implemented in single-arm prospective studies with pharmacokinetic endpoints, no randomized trial with recurrence or survival endpoints has been completed (a first phase 3 randomized trial of personalised dose optimisation, POWER, NCT07502820, opened in April 2026 with estimated completion in 2036).
Background
Tamoxifen is a selective estrogen-receptor (ER) modulator used for decades as adjuvant endocrine therapy in ER-positive breast cancer, i.e., tumors whose growth is driven by the estrogen receptor. Tamoxifen is a prodrug: it is converted mainly by the cytochrome P450 enzyme CYP2D6 (a highly polymorphic liver enzyme whose activity differs between individuals because of inherited gene variants) into (Z)-endoxifen, a metabolite roughly 100-fold more potent than tamoxifen itself at blocking estrogen-dependent tumor growth. Because CYP2D6 activity, drug–drug interactions (e.g., with certain antidepressants), and other patient factors vary widely, steady-state endoxifen plasma concentrations span a large range on the standard fixed dose of 20 mg once daily, and about 22% of patients fall below a proposed minimum effective endoxifen concentration of (equivalently about 16 nM). This threshold derives from two retrospective cohort analyses — Madlensky et al. 2011 and Saladores et al. 2015 — which reported ~26% lower breast cancer recurrence and better distant relapse-free survival in patients whose endoxifen exceeded it; it has never been confirmed prospectively. Therapeutic drug monitoring (TDM, dose adjustment guided by measured drug concentrations) and model-informed precision dosing (MIPD, dosing guided by a mathematical pharmacokinetic model of the individual patient) are the two proposed strategies to keep patients above the threshold. The source work (Klopp-Schulze et al. 2020) pooled data from six clinical studies (468 patients, 3,554 concentration observations) into a nonlinear mixed-effects (NLME) population model — a statistical framework that simultaneously estimates typical population pharmacokinetics and partitions variability into interindividual variability (IIV, differences between patients, expressed as coefficient of variation, CV), interoccasion variability (IOV, within-patient variability between dosing periods), and residual error. That analysis quantified that CYP2D6 activity, CYP3A/CYP2D6 drug–drug interactions, and age explain much of the between-occasion variability, but a large IIV in endoxifen formation (47.2% CV) remained unexplained, and simulations showed MIPD (using doses from 5 to 120 mg once daily plus three TDM samples) would reduce the fraction of patients at risk of subtarget endoxifen from 22.2% (standard dosing) to 7.19%. The authors explicitly identified as required future work: (1) establishing the endoxifen exposure–response relationship for breast cancer recurrence and mortality, for which adequately powered prospective trials are needed; (2) prospective demonstration of safety and efficacy of individualized, partly off-label dosing; (3) standardized, validated bioanalytical methods for tamoxifen and endoxifen across centers; and (4) evaluation of the MIPD workflow against standard care in a real-world prospective trial with clinical endpoints. For the exposure–response question the source specifies carefully designed prospective studies jointly assessing pharmacokinetics and clinical endpoints (breast cancer progression/recurrence, adverse drug reactions) and/or an appropriate biomarker (e.g., tumor size, circulating tumor cells, Ki67 antigen over time) in a representative, well-defined tamoxifen patient population, and it records the urgent call made during the exposure–response controversy for a well-designed prospective, randomized clinical trial with sufficient power. The unexplained residual pharmacokinetic variability (39.9% CV on tamoxifen elimination, 47.2% CV on endoxifen formation) remains a feature of the analyzed data. The questions posed below concern the prospective validation of the endoxifen exposure–response relationship and the clinical utility of model-informed precision dosing. A separate open question is whether (Z)-endoxifen supplementation is superior to model-informed precision-dosed tamoxifen with respect to clinical outcomes.
Problem Statement
As stated in the source work and still unresolved: (1) Propose the prospective randomized study design, analysis plan, and power calculation that would test whether achieving endoxifen plasma concentrations above the proposed therapeutic threshold (, ~16 nM) reduces breast cancer recurrence and mortality in patients receiving adjuvant tamoxifen, i.e., that would validate or refute the endoxifen exposure–response relationship that individualized-dosing strategies presuppose. The study must jointly assess pharmacokinetics and clinical endpoints (breast cancer progression/recurrence, adverse drug reactions) in a representative, well-defined tamoxifen patient population; a biomarker-only design (e.g., tumor size, circulating tumor cells, Ki67 antigen over time) is not sufficient. (2) Specify the prospective randomized design, analysis plan, and power calculation that would establish whether model-informed precision dosing of tamoxifen — individual dose selection from the population pharmacokinetic model using the patient's CYP2D6 activity score together with endoxifen therapeutic drug monitoring samples — improves outcomes compared with the conventional standard fixed 20 mg once-daily dosing in routine oncology practice, assessed with clinical endpoints and long-term safety monitoring. Each power calculation presupposes a primary clinical endpoint and an assumed effect size (or event rate) grounded in the cited literature. The population pharmacokinetic model, the definition of the CYP2D6 activity score, and the score-to-dose rule are not fixed and must be selected and specified by the solver.
